This procedure is always performed using implants. The implants are made of silicone shell which can be filled with silicone gel or cohesive silicone gel.
Most implants contain silicone gel, as this substance provides best durability and malleability.The reputation of silicone gel has been fully restored, contrary to some claims, it has been proven not to cause rheumatism or breast cancer and implants do not rupture at high altitudes.
A roughened, texturised surface on the silicone shell is preferable by some surgeons to a smooth surface, as it was thought to cause less capsular fibrosis. There are round, oval and dropshaped implants. The latter correspond most to the natural female anatomy and also looks natural in a horizontal position. Standard implant size ranges from 60-600 cubic centimeters. The most popular implant type is round design.
The operation, which takes one to two hours, is carried out when the patient is fully anesthetized or a “twilight” anesthesia has been administered. An incision approximately four centimeters in length is made in the skin, usually either in the armpit (axilla), below the nipple or in the crease under the breast. The cavity for the implant is prepared through the incision: the implant can be placed either behind the mammarial gland (sub-glandular position ) or behind the pectoral muscle (sub-pectoral or sub-muscular position). The latter is opted for by 80 percents of surgeons.
Technique
The implant is placed in a pocket either directly behind the breast tissue or underneath the pectoral muscle which is located between the breast tissue and chest wall.
The method of inserting and positioning your implant will depend on your anatomy and your surgeon's recommendation. The incision can be made either in the crease where the breast meets the chest, around the areola (the dark skin surrounding the nipple), or in the armpit.
