Rhinoplasty (Nose Surgery) Rhinoplasty otherwise commonly known as a “Nose Job” is the surgical reshaping of the nose.


Various procedures are available to alter the appearance of the nose or improve the nasal function. The goal is to improve the appearance of a facial structure that is already normal. Decision for surgery must be carefully made to ensure that the gains are worth the risk of complications or a poor result. Careful discussion and communication is established between you and the surgeon to define goals, outline a plan, and identify limitations of surgery. Decision for surgery is made after complete understanding that the gains are worth the risks of treatable complication or failure to achieve unrealistic expectations.



Techniques of the procedures

The incisions are placed on the rim of the alar of nose. Soft tissues of the nose are separated from the underlying structures, the bone and cartilage are then reshaped and the skin re-draped over the surface. Patients who have a deviated septum may benefit by correction of this problem at the time of surgery. In a typical Rhinoplasty, the nasal bones, upper lateral cartilages, and lower lateral cartilages are altered, to achieve the desired result.

Other corrections of the nose use varied techniques  as applied to achieving a contoured, well-projected tip, corrections of nose deviation and/or septal deviation, nose augmentation, and nose re-shaping or reduction.

Commonly required modifications of the tip complex include the following:

  • Change in tip projection
  • Decrease in the distance between the tip-defining points

  • Decrease in tip fullness

  • Creation of a supratip break

  • Adjustment in the relationship between the columella and alar rim

  • Alteration of tip rotation

  • Lengthening or shortening the nose by repositioning the entire tip complex cephalically or caudally

The operation is done under either local anesthetic (you are awake) or general anesthetic (you are asleep). Most patients are happier when they are asleep. You can choose.. Most of the time this is an outpatient operation and a stay in the hospital is not needed.



Rhinoplasty Consultation

During consultation the surgeon plans the procedure considering the overall nasal configurations i.e. balance, nasal and facial symmetry, and proportion of the entire nose, nasal tip, nasal bones and septum. In some cases, the airway may be obstructed because of septal deviation or internal or external valve soft tissue collapse. All issues will be considered, discussed and decisions included in the plan of surgical approach.


After a Rhinoplasty - What to Expect

The nose will be covered with either plaster of Paris, or a light metal or plastic splint to maintain the position of the nasal bones. A cool compress is applied to the eyes for the first 24 hours of surgery, to reduce swelling and ecchymosis. Antibiotic prophylaxis is given and pain, if present, is controlled.



What Are the Complications and Side Effects of Rhinoplasty?

When Rhinoplasty is performed by a qualified plastic surgeon, complications are infrequent and usually minor. Nevertheless, the possibilities always exist and these include infection, nosebleed, or a reaction to the anaesthesia. You can reduce your risks by closely following your surgeon's instructions both before and after surgery.

After surgery, small burst blood vessels may appear as tiny red spots on the skin's surface; these are usually minor but may be permanent. As for scarring, when Rhinoplasty is performed from inside the nose, there is no visible scarring at all; when an "open" technique is used, or when the procedure calls for the narrowing of flared nostrils, the small scars on the base of the nose are usually not visible.

Recovery

Most Rhinoplasty patients are up and about within two days, and able to return to school or sedentary work a week or so following surgery. It will be several weeks, however, before you're entirely up to speed. Nose blowing should be avoided for 3 weeks after surgery.  Avoid bumps to the nose for about six weeks. Eyeglasses should be taped to the forehead, off the bridge of the nose, for the first month. Exercise is abstained for 3 weeks postoperatively and contact sports can be resumed 3 months after. Avoid strenuous activity (jogging, swimming, bending, and sexual relations-any activity that increases your blood pressure) for two to three weeks. Avoid hitting or rubbing your nose, or getting it sunburned, for eight weeks. Be gentle when washing your face and hair or using cosmetics.



Results

Your nose will begin to look better and your spirits will improve. Within a week or two, you will no longer look as if you've just had surgery.



Pre-operative precautions:
  • Inform us about any allergies, any serious medical condition(s), and all medications you are taking (both prescription and non-prescription).
  • Avoid aspirin and aspirin-containing medicines for two weeks prior to surgery.
